Report -> Admin activity shows "Creating User" via SCIM, user not in User list.

This is a returning employee, She was disabled in AD and then enabled. It shows multiple attempts to recreate her LastPass user. What do I need to do to fix this?

I've tried manually creating the user etc. with and with the leading *. 
Each attempt the user is still prompted for a password.
