re-enable multifactor

I had an employee loose their phone so I disabled multifactor authentication for them. The policies for that user include "Require any MFA option after grace period" and "Require use of any MFA option". However, no MFA device is shown for that account and I see no way to re-enable multifactor. How do I do that?
