I have LastPass premium running on my Mac Mini, iPad, and iPhone. Just upgraded iPhone from 8 to 12. For some reason I can't pair the DFA app to the new iPhone. So, I can't get into the vault on the iPhone because the SMS text doesn't come through to verify my access. When I try to open LastPass on the new iPhone it asks for the SMS push notification but the push notification never comes through. The "use alternate verification" option is grayed out and I can't use it. In other words, in order to pair the DFA app to the new iPhone so I can access my account, I have to get a code via the DFA app. In order to get the code via the DFA app I have to pair the DFA app with my new iPhone. In order to pair the DFA app with the new iPhone I have to get the code via the DFA app. It is an endless loop from which I cannot escape.

I see that LastPass Authenticator is enabled for your account. Could you try going to your Multifactor Options on a different device that is already logged in to LastPass to disable the LastPass Authenticator, then sign in on your iPhone and then re-enabling LastPass Authenticator to see if you then able to get verification codes via the LastPass Authenticator app?
I'm having the same issue. I am able to access LastPass on my other iphone, but I don;t see an option in the LastPass app to disable LastPass Authenticator. How do I do that?
I was able to login to my LastPass account on another device and disabled the MFA option. I then deleted the MFA app from my new iPhone. With MFA turned off, LastPass works fine on my new phone. I haven't had the time to reinstall the MFA app and turn MFA back on.
