Multifactor authentication failed bug

I'm unable to login to my vault due to a bug with 2FA.


Every time I try to login with my email+password an error message pops up saying "Multifactor authentication failed".  Any method of account recovery gets stuck on the same issue. I still have my password, my Google Authenticator codes and my phone number is linked, but no matter what recovery process I try I get stuck on the 2FA error.

 

I suspect Duo security breaks it, can anyone from support help me disable it on my account?

Yes, tried forgotten password and account recovery, but still got stuck

 

I finally managed to resolve my issue, after login attempts the app showed for <1 second an option to select a different 2FA provider (before the Multifactor error appears). So after a few tries I was fast enough to click and cancel the Duo autopush request and switch to Authenticator TOTP codes.

Unfortunately, I couldn't rely on the Last Pass Authenticator, I moved on to Authy. Haven't had an issue since. Google you need to back up your accounts, or have it on another device too, or you lose all your data. I still have last pass with about 3 accounts (for testing) and they have all been working again. I'll probably convert back, but not right now. I don't remember what I did to fix it because I did so much to troubleshoot it. If I recall, I turned thank all off in each specific accounts, then deleted them all from last pass and started fresh ones. Hope this helps?

I'm not talking about the Authenticator apps, I simply can't login to my Lastpass vault at all.

 

This is what shows after entering email and master password:

This seems to be an issue with the Duo 2FA integration, so I need to disable Duo in my vault settings.  But I can't access my vault because I wasn't logged in anywhere anymore (so account recovery also doesn't work):

Oh, sorry, I misunderstood you. Did you try the forgot password, or any other option, like send a 1 time code to your cell number or back up email... Try sending an email to last pass.

I am receiving the same error.  I have used Duo for MFA since the beginning of my account (2016).  I never get a prompt or a push or any other option to try anything or reset MFA.  Just no... you can't login.  I need MFA disabled on my account so I can get in to reset it.  I have it still working on my mobile app, but need to be able to login.  Also, since I cannot login, I cannot create a support ticket.  Not great.

Dear all, I have this same problem, but I can't find the solution here. Two factor authentication keeps failing. How do I regain access to my account and vault?
