The PIN Code is an option that can be enabled in the LastPass App settings, this can also be an option that is required if your LastPass account is a LastPass Enterprise account (a work account) and your admin requires a PIN Code to be used. This pin code would have been set by you when this option was enabled or when logging in to the app for the first time if your LastPass account is associated with an Enterprise plan and this is an option that your admin requires. If you don't remember the PIN code you could try uninstalling the LastPass app and reinstalling to see if you can then log in to the app with your username and master password and no PIN code.
