On Mac OS, when I am using Safari and have the LastPass app installed on the icon bar within Safari, whenever I click on the " . . . " icon to have the LastPass options menu appear, the menu box will show just an empty box. Half the time it will not display the menu options. This happens in Safari on multiple machines running Mac OS. The LastPass extension does not exhibit this behavior for the other browsers on Mac OS, likely because they are installed differently. This problem with LastPass for Safari has been going on for many months now.

I've been having the same problem since installing LastPass version 4.94.0. This problem does not appear in version 4.93.0 nor in version 4.89.0. I've downgraded to the older version for now until it is fixed.
The blank window is a prompt for Multifactor Authentication (MFA).
I downgraded to version 4.86 and the same window popped up, but wasn't blank.
There are 2 solutions for this issue currently.
1. Downgrade
2. Disable MFA

Same here, 4.94 on Safari. This compounds the other nagging issue on Safari that I've been experiencing since the 4.93 update, where I cannot autofill by clicking the LastPass icon in the username or pw boxes on a site; I have to two-finger click and choose "Fill [sitename]." This is really frustrating. Yes, I use TFA and no I'm not disabling that. 1Password looking more attractive by the day...
