...the problems persists with Google Authenticator, and is gone with LastPass Authenticator.

LP Version 4.95 seems to work for me (but there were some issues at first, see below). I am still using Google Authenticator and didn't have to make any changes. Safari Version 15.4 (16613.1.17.1.13, 16613) MacOS Version: 11.6.5.
After I updated LP, it worked fine immediately. But then after I logged out of LP and tried to login through the extension, it just won't do anything. The window was not blank. It will accept password but still showed login menu asking for password again. It seemed like it wasn't connecting at all. So I disabled the extension (Safari->Preferences, uncheck Last Pass) and then re-enabled it. Then it started working. Keeping my fingers crossed... and hoping it was a temporary glitch.
After a reboot, I still got the blank window with a dash in it. Then I selected to log out of the app, quit it, quit safari, opened LastPass app, and logged in. I'm not optimistic that this will last, but it's now filling passwords for me in Safari, and the plugin button is no longer for decoration. LP app was still showing me the "Upgrade" button, and said I was a Free User, but after editing a password, it now displays correctly that I'm a Premium User.

I don't know that I'd call this fixed, but it's at least less broken now.
This process seems to be working for the most part:
1) Download LastPass for Safari Manually (which is not an obvious link - look for "Download" in tiny print at the bottom)
2) Quit LastPass
3) Quit Safari
4) Run AppCleaner (note: preferences are deleted)
5) Install LastPass
I haven't figured out all of quirks, but it's nearly back to normal. I did turn back off "Automatically fill login information" - I don't know why this would be on by default.
In Safari, the Caution symbol seems to be up most of the time now. Here's what the Support site has to say about it. Note that the Caution symbol appeared on the Lastpass support site too!

I was getting a blank box when LP found the new update. That's gone after updating.
NEW issue... when I launch LastPass, I'm getting double-boxes for the authentication...
With the new "fixed" version of LP, new issue has developed. In some website login screens, the LastPass icon no longer appears in the Username/Password fields. And going up to the menubar and selecting the login credentials does not populate the User/password fields. I literally have to cut and paste to get that info in the fields.
@asfesq : I also bumped into this, but I think this is how it's supposed to work. The MacOS app and the Safari plugin are "two devices" as far as LastPass is concerned. The one in the back in your screenshot belongs to the Safari plugin, and the one in the front belongs to the MacOS app.
If you decide for both to "trust device" then they end up in the Trusted Devices list separately. The suggested device names from LastPass also differ: "Mac OSX" for the Safari plugin and "Webkit Mac OS" for the MacOS app.
I just played around a bit with LastPass 4.96 using Google Authenticator (MacOS 12.4 / Safari 15.5):
