I do not have this problem with Chrome, but in Safari (latest version, latest OS X), I consistently get prompts from LP to save a password for the wrong site.
1. Visit site (ex. bank).
2. Login and use site.
3. Go to another site that requires a login.
4. It will prompt you to save the password as new for the site visited in #1.
This has been around (for me) for a couple of versions and only with Safari. Are you aware of this? If not, is there anything I can do to help you fix it? I prefer using Safari on my MBP.
