I submitted a support request for these two issues. I have seen that many others have similar issues, so I will share my support request and any possible resolutions here for the community.
Issue 1) Android continuously request app permissions that have been granted (repeatedly.) This is not an isolated issue - the support forums have many posts about this. Here is just one:

I posted the following to my own post about this:
- 'I get an annoying reminder on my Android concerning App Fill permissions every time I log in to LastPass. I have enabled this permission, done a complete uninstall and reinstall, and re-enabled the permissions. The warning persists. Samsung Galaxy 9+and Android 10. This has been occurring since Android 9 (at least!)'
Issue 2) Also, Lastpass is not filling a lot of sites even with autofill enabled. This occurs on almost every site on Android, but also on many sites. (Windows 10 Pro 64-bit Version 20H2) and Chrome, Vivaldi, Firefox, and Internet Explorer. (I refuse to use edge.)
Although I do have an ad blocker plugin (uBlock Origin), this occurs even when I have disabled it and reloaded the page.
LastPass will fill my password, but NOT my username. Every time, I must copy and paste the username. This has been occurring for over a year.
- I have completely removed LastPass from my Android phone, restarted, re-installed, and re-granted all permissions, but that problem persists.
- I have uninstalled the LastPass browser plugins and application on Windows, cleared all browser history & caches, rebooted, and re-installed, but the problem persists on Windows, also.
I will update this post with information from support when and as I receive it.
