I also get this. My Last Pass Authenticator app appears to require restoring from backup frequently. It feels like daily. I use an iPhone and I have deleted the app and reinstalled it - this hasn't fixed the issue.
Could you try completely uninstalling both the LastPass password manager and the authenticator apps from the app store, restarting your device, then reinstalling both apps to see if this issue continues?

[I just tried to login to lastpass using lastpass Authenticator. I approved it on my Apple Watch and nothing happened. I tried a few times. I grabbed my phone which was charging ac market, and opened lastpass Authenticator. It wouldn’t show me my codes when I opened it. It said “restore from backup” or “add a new code”. There was no way out of the prompt. I closed the app and opened it, still there. I clicked restore. Now all my 2FA logins are gone. Where did they go? I need them restored.]
Hi, I also faced that issue, I'm iPhone 11 pro Max user, however I fixed it reinstalling the app.
@RachelO It doesn't help. No, I'm good, I don't need to file a report, I've already lost the codes and spent hours recovering what I can. That data is gone and you can't get it back. You guys can find your own bugs and file your own reports. I'm already not using LastPass Authenticator anymore.
I submitted a bug report on this and got an absurd “please go away” answer saying that the problem was in iOS 15 when I specifically said this same problem occurred in iOS 14.
this is a serious bug… people losing their Authenticator accounts. Fix it.
